ADMISSIONS

Our Admissions Process

I.S.59Q is a NYC Department of Education middle school that accepts students on a rolling basis throughout the year.

Interviews are conducted for our Rising Star Academy Gifted program.

Please contact the I.S. 59Q Parent Coordinator, Mrs. Leverett, at SLeverett@schools.nyc.gov to receive the Rising Star Academy application.


When deciding to enroll at I.S.59Q, please contact our secretary of students, Ms. Tate, at PTate2@schools.nyc.gov.
 
To prepare for enrollment, please have the following documents readily available for electronic distribution. If electronic distribution is not available to you, please go to our contact page and call the school's main office, during normal business hours 8am - 2pm, Monday - Friday, for more information and guidance.

NOTE

If you need a copy of your child’s birth certificate, you can order it online through the New York City Department of Health and Mental Hygiene. Please visit the Office of Vital Records webpage for more information.

Forward all correspondence to

Ms. Tate, Secretary of students.

We shall process your admission and update you

 within 5days of receiving all documentation.

THREE (3) PROOFS OF RESIDENCE

​

- A residential utility (gas or electric) in the resident’s name issued by National Grid, Con Edison, or the Long Island Power Authority dated within the past 60 days.

- Documentation or letter on letterhead from a federal, state, or local government agency, including the Internal Revenue Service (IRS), City Housing Authority, Human Resources Administration (HRA), Administration for children’s Services (ACS), or an ACS subcontractor indicating the resident’s name and address; must be dated within the past 60 days.

- An original lease agreement, deed, or mortgage statement for the residence

- A current properly tax bill for the residence

- A water bill for the residence; must be dated within the past 60 days

- Official payroll documentation from an employer, such as a form submitted for tax withholding purposes, or payroll stub; must be dated within the past 60 days. Please note a letter on the employer’s letterhead WILL NOT be accepted.

- Child’s latest report card/transcript

- Child’s individual Education Programs (IEP) and/or 504 Accommodation Plan (If applicable and available)
